1. If you have concerns about your child's needs and development in regards to speech, occupational, and/or physical therapy, please contact the child's pediatrician's office and request a referral be sent to our office.

2. Our office will call you to set up an evaluation once we receive your child's referral. You may call our office to request the status of the referral. 

3. Be on the look out in your email for intake forms from our office to fill out prior to your arrival for the evaluation. If you do not see this email, you do not have an email, or you need assistance filling out the forms, please arrive to the clinic that the evaluation is scheduled at approximately 20 minutes earlier than your appointment time. We will be happy to assist you.  

4. Evaluation day - please have a copy of your driver's license and the child's insurance card ready to be presented to our front office staff upon arrival. The intake form must be completed before the evaluation may begin. 

5. An evaluating therapist will conduct a thorough evaluation based on reported concerns and the physician's referral. An evaluation takes approximately 45 minutes to an hour. 

6. Once the evaluation is complete, the evaluating therapist will write a report based on the findings of the evaluation as well as a plan of care. Our office staff will work with you to schedule recurring treatment appointments, if necessary, either in person immediately following the evaluation, or they will give you a call shortly after the evaluation.
